Peer Buddy Observation and Evaluation Forms

The Peer Buddy Evaluation Form is a tool that may be used to document the provision of adequate supervision, training, and feedback to students serving as peer buddies. The form documents (1) the date of the formal observation, (2) teaching techniques used by a peer buddy, (3) general interactions with the partner, and (4) feedback and recommendations given the peer buddy. Complete the form while conducting a formal observation of the buddy carrying out one or more assigned responsibilities. We then suggest that you use this form to give the student feedback on their performance at your next supervisory meeting. You can document feedback and recommendations in the space provided and on the back of the form if needed.

It is recommended that observations and feedback sessions be conducted at least once a month with each peer buddy. The Observation Schedule is a chart developed to help the supervising teacher monitor the number of observations and evaluations completed on each peer buddy assigned to the classroom.
